Team and Individual Picture Order Form

Coach Mike is scheduled to take Stingrays Swimming's team picture on Saturday, October 3 at 11:30 am at Mountain View Aquatic Center.

For TEAM pictures, please arrive no later than 11:30 am to receive your team T-shirt and set-up for the team photo.

INDIVIDUAL and/or buddy pictures are taken at your practice location the following week (with the exception of the senior group - see below). The schedule for your facility follows.

Please complete the form below for team and/or individual pictures.

All orders are 100% refundable if not completely satisfied.

Please come prepared for your pictures dressed in either your Stingrays Spiritwear or swimsuit (with or without cap/goggles) on the appropriate day for your practice pool. Don't forget to bring your smiles!

PHOTO RELEASE

I hereby grant Coach Mike Photography permission to use my child/children's likeness in a photograph for purposes of marketing and/or publicizing the Coach Mike Photography business in any and all of its publications, including website entries, without payment or any other consideration, and I irrevocably authorize Coach Mike Photography to edit, alter, copy, exhibit, publish or distribute this photo for said purposes. I hereby waive the right to inspect or approve the finished product, including written or electronic copy, wherein my child/children's likeness appears and also any right to royalties or other compensation arising or related to the use of the photograph. I understand and agree that these materials will become the property of Coach Mike Photography and will not be returned.

I hereby hold harmless and release and forever discharge Coach Mike Photography from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I am 21 years of age and am competent to contract in my own name and on behalf of the child/children being photographed. I have read this release before acceptance and I fully understand the contents, meaning, and impact of this release.
