Summer Open Room Change Request

Please fill out this form in its entirety. Once submitted you will be randomly scheduled for an appointment starting at 8:30am onMay 19, 2014. You will be notified of your appointment via HuskyMail and will have the opportunity to change your appointment time if the one scheduled for you does not work. You will also have the option to call in to your appointment as you do not need to be physically present at that time. During your appointment you will be able to discuss current availability in the halls and possibly secure a new space.

The cut-off date for submissions to be in the first round of appointments is 12noon on Friday, May 16, 2014. This form will remain open after that point; however you will be scheduled on a space availability basis.
