The following application form is provided for the purpose of securing a Letter of Support from Cricket Australia for the purposes of securing a Temporary Activity Visa (subclass 408).
The form will ask for the following information:
- Details of the Sponsor (club name, contact name, address, email address)
- Details of the Applicant (player name, address, email address)
- Further Details about the Applicant - role to be performed, letter of recommendation, ESPN Cricinfo ID
