Possible Risk Disclaimer

While Focus Learning staff and instructors will make every reasonable effort to minimize exposure to known risks associated with each camper’s participation in our camp, I acknowledge that my child may be required to participate in various physical activities that may involve risk of injury. I hereby permit the camper to participate in the full range of program activities, except as specifically noted by me in the health information section of the registration form.

Should it become necessary for my children to have medical care, I give permission to the teacher to use his/her best judgement in obtaining the best service for my child. I understand that any costs will be my responsibility. I also understand in the case of accident or illness, I, and/or my emergency contact, will be notified.

By signing this waiver, I permit my child to participate in the full range of Program activities, except as specifically noted by me in the health information section of the registration form.

Off Property Consent Form

By signing this waiver, I give my consent for my child(ren) to participate in the off-property excursion, which involves walking beyond the school property under teacher supervision.

Payment/Cancellation Policy

• Children may not be permitted to attend camp until full payment has been received or processed.
• 80% of the camp fee will be refunded on cancellations received 10 or more business days prior to the start of the camp session. Refunds will only be honored on full camp payments.
• 50% of the camp fee will be refunded on cancellations received less than 10 business days prior to the start of the camp session. Refunds will only be honored on full camp payments.
• There will be no refunds issued on or after the first day of camp. Cancellation due to unexpected emergencies is subject to a minimum of $20 per week per person administration fee.
• Prorated refunds are not available for days absent. Campers can attend other camp programs as a makeup if there are spots available.
• Transfers may be made up to two weeks prior to the start of a session providing there is available space in the desired program.
• Cancellation and transfer notice must be submitted in writing.
• Focus Learning reserves the right to cancel a camp with insufficient number of campers and will offer a full refund as a result. At least 2 weeks of notice will be provided.
