HAWORTH APPAREL/MERCHANDISE ORDER FORM

The Haworth Home and School Association (HHSA) is a 501(c)3 organization run by the parents of the Haworth Public School. Its primary goal is fundraising in order to enrich the education of our children with programs and equipment/supplies. All of the profits earned from the sale of Haworth branded apparel goes to the HHSA.

Please follow this procedure for ordering items:
