The undersigned participant, along with their parent or legal guardian, acknowledges and agrees to the following terms and conditions for participation in the Fountain City Flag Football: 1. Assumption of Risk I, the parent or legal guardian of the above-named participant, understand that participation in any athletic event involves inherent risks, including, but not limited to, physical injury, accidents, or even death. I acknowledge that the participant will be engaged in activities with inherent risks and accept full responsibility for any injury or harm that may result from their participation. 2. Medical Treatment Authorization In the event of an emergency, I give consent for the Event staff, athletic trainers, or emergency personnel to seek medical treatment for the participant if deemed necessary. I understand that I will be responsible for any medical costs associated with any injury or illness sustained during the Event. 3. Waiver of Liability I, for myself and the participant, release, discharge, and hold harmless the organizers, sponsors, staff, volunteers, and any affiliates of the Fountain City Flag Football from any and all claims, liabilities, demands, or actions arising out of or relating to the participant’s involvement in the Event, including any damages or losses that may arise as a result of negligence or misconduct on the part of the Event organizers or other participants. 4. Code of Conduct I agree to encourage the participant to act respectfully toward others, play fairly, and abide by the rules and instructions provided by Event staff. I understand that any conduct deemed inappropriate by Event staff may result in the participant's removal from the Event without a refund. 5. Photography/Media Release I understand that the Event may be photographed, filmed, or otherwise recorded. I grant permission for the participant’s image, likeness, or performance to be used in promotional or commercial materials by the Fountain City Flag Football or its affiliates, without compensation. 6. Uniforms and Sponsorships. I understand that uniforms may feature logos or images of sponsors, and I consent to the participant wearing such uniforms during the Event. By checking the box below this waiver, I acknowledge that I have read, understood, and agreed to all the terms outlined above and grant permission for the participant to participate in the Fountain City Flag Football under these conditions. *
