By completing this form you are indicating that you wish to petition charges or cancellation fees for a specific semester. If you fail to complete the form completely it will be returned to you, and not submitted for review. Once this form is completed it will be automatically submitted for review. Review will take about 2 weeks if the form is complete and all necessary documentation is attached.
