RELEASE OF LIABILITY In consideration of the permission granted by Team Canada Dance and the donated studio usage for auditions, rehearsals and its agents (including the Director, choreographers, chaperones, studio owners, CNDT, CNDC, CDO, IDO, and Team Canada Dance Hip Hop, Streetdance, Tap, Ballet, Jazz, Modern, Showdance, Acro, Commonwealth teams, World Dance Teams, Team Canada Dance and/or Team Canada Dance Org) for allowing the registered dancers and parent(s), or guardian(s) or participants agree with the follows A. That neither I/We nor the Participant shall hold Team Canada Dance Org. and their directors or choreographers liable for any loss, damage, expense, injury, or death whatsoever, suffered or incurred by the Participant arising out of, or in any way attributable to the activities inherent in Team Canada Dance participation and the IDO World Championships, or the use of the Lands or any vehicle before, studio, rehearsals, during or after his or her participation in Team Canada Dance activities. B. To be involved in Team Canada Dance auditions and rehearsals and travel C. That Team Canada Dance, Canadian National Dance Championships, Dance Studio, the rehearsal studios or their owners, choreographers and staff, Team Canada National Director Bonnie Dyer, Assistant Directors Alison Sharp and Melissa Dyer, International Dance Organization, Competition Organizers, or any of the Team Canada Dance Choreographers of which are not liable for any accidents or injuries or cancellations of any events in connection with World Dance Championships. D. I/We understand that there is an element of risk involved in some activities engaged in by Team Canada Dance, as is the transportation to and from these activities. I/We accept these risks on behalf of the Participant. E. That Team Canada Dance has a right to expect the Participant to abide by the rules set out by Team Canada Dance for involvement in the program, and if the Participant does not abide by these rules he or she may be sent home at my/our expense and without any reimbursement of program fees paid by the Participant. This includes behaviour and social media rules.
WAIVER/RELEASE As the legal parent or guardian, or dancer over the age of majority, I agree to allow and give permission to the Team Canada Dance and affiliated competitions to use any photos or video footage of my child/children or myself (ie. auditions, rehearsals, competitions and/or performances) for the promotional use and purposes, should Team Canada Dance choose to do so in regards to the advertising of Team Canada Dance or the affiliated competitions and performances. You may contact us for exceptions.
SOCIAL MEDIA POLICY We encourage members to share their experiences through different social media. We encourage responsible use and contact us if you have a problem or see a problem on another page. Please DO NOT create any social media accounts in our name. You may tag us in your posts. DO NOT upload/share any choreography before the competition. Please note that this policy may be updated at any time.
MEDICAL EMERGENCIES As the legal parent or guardian, or dancer over the age of majority, I permit Team Canada Dance, its owners, operators, and choreographers, to seek medical treatment for the participant or myself in the event they are not able to reach an emergency contact. I hereby declare any physical/mental problems, restrictions, or conditions and/or declare the participant to be in good physical and mental health.
COVID/VACCINATION POLICY We are not liable for any COVID-19-related risks. Dancers/travellers are expected to keep up with travel entry requirements including vaccination and testing that could be subject to change at any time at their own risk
ONLINE REGISTRATION & PAYMENTS By registering online, I agree that the information is correct and payments will be made as directed in your Travel Package and paid in full by the date given in your Travel Package and Team Handbook. Credit Card payments are not an option. Acceptable methods of payment: E-transfer or deposit into the Team Canada Dance Org. RBC account directly. You will be charged the deposit upon registration for each traveler, with no annual membership fee. Please contact the Team Canada Dance administration for other payment methods. Please note that we do not share your financial information with any third party.
REFUND POLICY There are absolutely NO refunds or transferals on any fees that have been paid to Team Canada Dance once travel bookings have been confirmed before departure. Refunds may be available (minus the $250 deposit/traveler upon registration) for emergencies and acceptability is determined by Team Canada Dance. Any cancellation fees charged by the airline or hotel are your responsibility. NO refunds for choreography, rehearsal, or costume fees paid to the choreographer, Team Canada Dance, and/or designer - which is not associated with these fees whatsoever. Refunds for auditions may be granted if notified 24 hrs in advance. The travel deposit is non-refundable.
WITHDRAWAL POLICY To withdraw myself or my child from any team commitments, including rehearsals and travel, I understand that I must notify TEAM CANADA DANCE ADMINISTRATION, in addition to THE CHOREOGRAPHER well in advance of Team Travel via e-mail. Simply not attending rehearsals does not justify withdrawing from the Team. If costumes have already been ordered, I understand that I am responsible for full payment of all costumes for all my dancer(s). Once airfare and hotels have been booked, I am responsible for any cancellation fees incurred. Please see the refund policy.
COSTUME FEES & CHOREOGRAPHY FEES Team Canada Dance does not collect costume or choreography fees including rentals, hall rentals, or props. All related fees are to be paid directly to the choreographer and fees are determined by the choreographer.
SIGNATURE As the legal parent or guardian of the dancer, or a dancer over the age of majority, I release and hold harmless Team Canada Dance, its owners, and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Team Canada Dance its owners and operators or in route to or from any of said premises.
I am aware of the risks connected with attending this event on this date. I agree to not hold accountable or bring legal action against the company, their officers, agents, or employees. This waiver releases the company from all liability relating to injuries that may occur on location before, during, or after activity.
By signing this agreement I agree to hold the company entirely free from any liability, including financial responsibility for injuries incurred, regardless of the reasons or circumstances.
I acknowledge the risk involved with this activity including, but not limited to, muscle tears, sprains, and other physical injuries. I certify that my participation is voluntary and I have been made aware of the risks. Additionally, I do not have any conditions that may increase my likelihood of injury.
I will make every effort to obey the company personnel, all safety rules, and will ask for clarification if needed.
